Immediate Action Required:

If any incident, hazard, or risk assessment occurs on-site, all contractors must immediately fill out and submit the required form.

Reporting Process:

The DMF Team (Maintenance & Projects) will receive the form.

If necessary, appropriate actions  appropriate actions will be taken immediately.

✅ Submission: Once the permit to work is submitted via Asana, the Facilities Manager will receive it immediately.

✅ Review & Approval: The Facilities Manager will check if the worker is permitted to start the works. Please allow 48hrs for this process. If required Urgent please contact DMF to arrange urgent review.

 

🚨Important Note:

⚠️No work should commence until approval is granted by DMF through an email or notification.

• Failure to comply may result in work delays or suspension.

Description

Hot Works Required for any work that generates heat, sparks, or flames, which could potentially cause a fire.It includes activities like welding, grinding, cutting, soldering, and using open flames or torches. Form Link
Asbestos /
Silica Works
Required for demolition or renovation of old buildings containing asbestos cutting, grinding, drilling, or demolishing materials that generate silica dust (e.g., concrete, bricks, stone), any works where asbestos or silica exposure could pose health risks. Form Link

Building Works
( Alterations & Additions )

 It allows construction, renovation, demolition, or other structural changes to take place legally. They ensure that the work complies with building codes, zoning laws, and safety regulations. Form Link

Confined Space Access

Required for workers to enter and work in a confined space. It ensures that all necessary safety precautions have been taken before entry. The permit is part of a permit-to-work system, designed to minimize risks associated with confined spaces, such as lack of oxygen, toxic gases, or hazardous conditions. Form Link
Demolition Works Required before demolishing a building or structure. It ensures that the demolition process follows safety, environmental, and legal regulations.  Form Link
 Electrical Works Required to ensure that electrical installations, modifications, or repairs comply with safety regulations and electrical codes. Form Link
Working at Heights Required when workers perform tasks at heights where there is a risk of falling as working on scaffolding, ladders, mobile platforms, roof and elevated platforms.It ensures that proper precautions are in place to prevent accidents. Form Link
Critical Fire Protection  Required for work involving fire protection systems, such as sprinklers, fire alarms, or suppression systems. It ensures that modifications, installations, or maintenance meet fire safety codes and regulations. Form Link

 Mechanical / Isolation Works

Required when workers perform tasks in mechanical activities such as installation, maintenance, or repair of machinery and equipment, involving works on rotating or moving machinery, opening pipelines or vessels containing hazardous substances. Form Link

Contractor -  Prestart / Risk Assessment Click It ensures workers understand and manage the risks associated with a particular job before they begin work.  Contractors must assess the work environment and processes to identify any potential risks or hazards. This ensures they are prepared for any challenges that might arise. Form Link
